Provider Evaluation Fee

• The Provider Evaluation Fee covers the clinical review process conducted by a licensed provider.
• This fee is charged per consultation and is non-refundable once the consultation has been initiated, regardless of approval outcome.

If You Are Not Approved

If a licensed provider determines you are not eligible for treatment:

• No medication will be dispensed.
• No additional charges will be incurred beyond the Provider Evaluation Fee.
• Any unused plan funds, if applicable, will be refunded.

Cancellations Before Consultation

If you request cancellation before a provider consultation has been initiated, you may receive a full refund.

Foundation Membership

Foundation Membership is refundable within 30 days of purchase, provided no membership-exclusive benefits have been materially utilized.
